Could someone pls give a step-by-step guide on how to create my own skill/item icons?
Any help would be appreciated.

You can't really get a step-by-step tutorial which generalizes icon-making. after all, it's art. It requires skill and practice to be able to make quality item icons.

Anyway, here are some basic techniques (which require a lot of patience, time and exercise to master):

1. Start by making the outern lines of the item (shape it).
2. Fill the icon with the appropriate colors.
3. Start butning the areas where the shadows are situated. You may want to combine it with smudge to acquire a subtle gradient effect between non-shadowed and shadowed area.
4. Now use dodge for the highlighted parts, combined with smudge as mentioned above.
5. Keep burning and dodging while adding more details to the item you are designing.
6. Add effects and decorations to the item (such as glowing runes, glowing aura, etc.).

Tip: You could start the icon at 192x192 but periodically zoom out to see how it would actually look at 64x64 so that you don't lose too many details during the final resize. You'd be surprised to see how catastrophical can shrinking sometimes be (even if it is only 2 times smaller).
